Welcome to our Insurance information page

Once you have your insurance information gathered please stop by the front desk at Hope Fieldhouse so we can make a copy of your insurance card and ID. 

Free Memberships Included at our Facility are:

Silver Sneakers

Must have 16 digit code provided by your Health Insurance

Active & Fit

Must have 8 digit code provided when purchasing Active & Fit or from your employer

One Pass (UCare)

Must have 10 digit Code that begins with A, B, or S

Renew Active (United HealthCare)

Must have 10 digit Code that begins with A, B, or S

Silver & Fit

Must have 8 digit code 

For all other Insurances :

For NIHCA Rewards and partial reimbursement

All other insurance members must purchase a Hope Fieldhouse membership and sign up on their own through the NIHCA for partial reimbursement from your Insurance.
